How Medicare works with your VA and TRICARE benefits

If you use the VA

VA health care and Medicare are separate. Medicare lets you use doctors and hospitals outside the VA, closer to home. Many veterans keep both.

Part B and penalties

VA health care doesn’t protect you from the Part B late enrollment penalty. If you’re turning 65, think it through before you skip Part B.

Plans without drug coverage

8 Nassau County plans for 2027 leave out drug coverage, and 7 of them have a $0 monthly premium. Many veterans pair these with VA prescriptions, and some include a Part B premium reduction.

TRICARE For Life

Military retirees need to keep Medicare Part A and Part B for TRICARE For Life to work. It pays after Medicare, so another plan often isn’t needed. We’ll tell you honestly.

Many Nassau County veterans get VA care at the Jacksonville North VA Clinic at 145 Heron Bay Road, just south of Yulee off I-95, which opened in 2024 with primary care, mental health, radiology, optometry and physical therapy. What’s new in Nassau County

Baptist Health’s Nassau Crossing campus on State Road 200 in Yulee, near Wildlight, opened an emergency and imaging center in late 2024 and added primary care, heart specialists and a GI practice in 2025. Baptist Medical Center Nassau remains the hospital in Fernandina Beach.

If you’ve started seeing a doctor at the new Yulee campus, check that the office is in the network of the 2027 plan you’re considering.

Returning plans

30 plans from 2026 are back in Nassau County for 2027, and 3 are new. 23 of them still have a $0 monthly premium.

Out-of-pocket limits

11 returning plans raised their yearly out-of-pocket limit and 2 lowered it. The typical limit went from $6,750 in 2026 to $6,950.

Monthly premiums

3 returning plans raised their monthly premium and 4 lowered it. The rest stayed the same. Even if your plan stayed, check your Annual Notice of Change.

Annual Notice of Change

Your plan is continuing but its costs or benefits change on January 1. Compare it with other 2027 plans before December 7.

Non-renewal letter

Your plan is ending or leaving your county. Write down the end date. You can choose a new plan October 15 to December 7, and you also get a special window from December 8 through the end of February.

Moved to a new plan

Your insurance company says it will move you to a different plan. You can accept it, but check its doctors, drug list and costs first, because they may not match what you have now.

If your plan is being merged into another plan, you’ll be moved to that plan automatically. Your letter tells you which applies.

What to have ready

  • Your letter. Your Annual Notice of Change or non-renewal letter, and your plan’s member card.
  • Your doctors. Primary doctor, specialists, hospital, labs and any planned procedures.
  • Your prescriptions. Each drug name, dose and how often you fill it, plus your pharmacy.
  • Your budget. What you pay now each month and for visits, and how much a bad year could cost.
  • Your VA or TRICARE card. If you have them, so we can see how your benefits fit together.

Questions from Nassau County residents

I get my care at the VA. Do I still need Medicare?

Many veterans keep both. VA care and Medicare are separate: Medicare generally doesn’t pay for care at the VA, and the VA generally doesn’t pay for care outside the VA unless it authorizes it. Having Medicare lets you see doctors and use hospitals outside the VA, such as in Fernandina Beach or Yulee.

Should I sign up for Medicare Part B if I have VA health care?

It’s worth thinking about carefully. VA health care doesn’t count as coverage that avoids the Part B late enrollment penalty, so waiting to sign up can cost you more later. We can walk through your situation at no cost.

Is there a Medicare Advantage plan that works with my VA prescriptions?

Yes. Nassau County has 8 Medicare Advantage plans for 2027 that don’t include drug coverage, which many veterans pair with VA prescriptions. VA drug coverage counts as creditable, so skipping a Medicare drug plan doesn’t trigger a late penalty while you have it.

I’m a military retiree with TRICARE For Life. What should I do?

Keep Medicare Part A and Part B, because TRICARE For Life requires both. TRICARE For Life pays after Medicare and already covers much of what Medicare doesn’t, so call us before changing anything. In many cases you won’t need another plan.

What happens if my plan is ending and I do nothing?

It depends on your letter. If your plan is not renewing, you’ll be moved to Original Medicare on January 1, without drug coverage unless you join a drug plan, and going 63 days or more without drug coverage can mean a lifelong late penalty. If your plan is being merged into another plan, you’ll usually be moved to that plan automatically, so check that its doctors, drugs and costs still work for you.
