Miami-Dade Medicare 2027: 54,021 people need to look at their plan this fall.

Miami-Dade has more people in changing Medicare Advantage plans than any other Florida county, and most of them also have Medicaid. Here is what that means for you, in plain English or en español.

Most of the change here is in Medicare-Medicaid plans

Miami-Dade is different from the rest of Florida. Of the 54,021 people whose 2026 plan isn’t coming back under the same plan number, about 31,800 are in plans built for people who have both Medicare and Medicaid (often called dual plans). Another 18,000 or so are in regular HMO plans, and about 3,900 are in plans for people with a chronic condition such as diabetes or heart disease. Almost none are in PPO plans.

If you have Medicaid too, your letter matters even more than usual. Some dual plans are being folded into another plan from the same company, and members are moved automatically. Others are ending, and you choose. Your Medicaid level (full Medicaid, or help only with your Part B premium) decides which 2027 plans you can join.

Full Medicaid

You may qualify for a plan that coordinates Medicare and Medicaid benefits together. People with both Medicare and Medicaid also get extra chances to change plans during the year.

Help with Part B only

If the state pays your Part B premium through a Medicare Savings Program, you may still qualify for some dual plans. We check your level before suggesting anything.

Medicare only

You can choose from the 41 plans open to everyone. Most are HMOs, so the doctor and hospital network is the first thing to check.

Choosing an HMO in Miami-Dade

Before you pick: check the network, not the extras

Most plans here are HMOs. That keeps premiums low, but you need a primary doctor in the plan’s network and referrals for many specialists. Plans in Miami-Dade often advertise extras like transportation, OTC allowances and dental. Those are useful, but they don’t help if your cardiologist or the hospital you trust isn’t in the network.

  1. 1Write down your doctors. Include the medical center or clinic group your primary doctor belongs to; many Miami-Dade doctors work through one group that contracts with only some plans.
  2. 2List your hospital. Jackson, Baptist Health, Mount Sinai, HCA and the UHealth system all have locations in the county. Know which one you’d want in an emergency.
  3. 3Check your medicines. Drug lists change every year, even when the plan name stays the same.
  4. 4Then compare extras. Only after the first three match does it make sense to compare dental, OTC and transportation.

What’s new in Miami-Dade

Jackson Health System broke ground on Jose Milton Memorial Hospital at UHealth Jackson, a 100-bed hospital in Doral off the Palmetto Expressway and Northwest 29th Street. In South Dade, HCA Florida Kendall Hospital added HCA Florida South Dade Emergency, a freestanding ER at 14190 SW 288 Street in Homestead.

Veterans can also use the Bruce W. Carter VA Medical Center in Miami. If you get care from the VA, tell us; it changes which plan type makes sense.

I have Medicare and Medicaid and my plan is ending. What do I do?

Read your letter first: it says whether your plan is ending or merging into another plan. If it’s ending, you can pick a new plan during Annual Enrollment (October 15 to December 7), and people with Medicaid also have extra chances to change plans during the year. Bring your Medicaid card to your review so we can see which dual plans you qualify for.

Why are so many Miami-Dade plans changing?

For 2027, about 54,021 Miami-Dade residents are in plans that aren’t offered again under the same plan number. Most of them are in plans for people with Medicare and Medicaid and in regular HMO plans. Companies redesign or combine plans every year; this year the changes are concentrated in those plan types.

Is an HMO or a PPO better in Miami-Dade?

Most plans here are HMOs, and they usually have lower costs if your doctors are in the network. A PPO lets you see out-of-network doctors for a higher cost, which helps if you split time between Miami and another state. The right one depends on your doctors.

How many plans can I choose from in Miami-Dade for 2027?

41 Medicare Advantage plans are open to anyone on Medicare in Miami-Dade, and 35 have a $0 monthly premium. Counting plans for people with Medicaid or certain conditions, there are 101.

I’m a veteran. Should I change plans?

Not necessarily. Medicare and VA care work separately. Some veterans pick a plan without drug coverage because they get medicines from the VA; others want a plan for care outside the VA. We’ll look at how you use both.
